The top of the document is print "USS FULTON (AS 11)/ WATCH, QUARTER, AND STATION ASSIGNMENTS." The next line has the date stamped "17 APR 1945." The two lines afterwards is filled out with the name "SATELO J. M.," rate "F 1/C," the bunk no. is left blank, division 8 section 1, and billet no. 8167. Under the section "BATTLE STATION," Condition 1: General Quarters Stations is "AMM. CARRIER TOP/HOIST FR84 STBD;" Condition 2: Watch & Watch Stbd-Port and Condition 3: Watch in Four-Sect are "Same as I (1)." The "DAMAGE CONTROL SECTION" is left blank. In the "EMERGENCY SECTION," Fire is "Qtrs. (Mach Shop)," Collision is "Qtrs.," and the raft number is filled out as "24FR68-P." The rest of the document is not filled out.
